ENROLLMENT AND REGISTRATION
INFORMATION

Children residing with parents or a legal guardian within the district are eligible to attend the North Bellmore Public Schools. Children who are residents will be admitted to school provided that they:

  are five years of age on or before December 1st of the year of entrance to Kindergarten (birth certificate required)

  have proof of all required immunizations

•  have proof of residency

Kindergarten registration is an ongoing process throughout the year but there are specific dates set aside for this purpose each winter for the coming year. Please continue to check the website and the local newspaper for the Kindergarten registration announcement which includes the dates and times for each school. This announcement is typically posted in December. Parents who have newly arrived in the community with children of Kindergarten age are requested to contact the schools immediately for Kindergarten registration.


To register for Kindergarten you must appear in person at your home school with the following:

  1. Original Birth Certificate or Passport

  2. Certificate of immunization from a physician that the child has received the following immunizations:

    • DPT or DTaP: 3 doses

    • Polio: 3 doses

    • Measles: 2 doses live vaccine, first after first birthday and second dose between ages of 15 months and six years.

    • Mumps: 1 dose live vaccine after first birthday

    • Rubella: 1 dose live vaccine after first birthday

    • Hepatitis B: 3 doses

    • Varicella: 1 dose after first birthday

Immunization laws were established in New York State to protect children from dangerous communicable diseases. Parents are required to provide the school with an immunization transcript signed by a physician and must include the printed name, address and telephone number of the physician or clinic/medical group.

Children in grades kindergarten, second and fourth and those transferring in, are required to have physical examinations. The physical exam can be provided by the family physician who must complete a form provided by the school. If this is not possible, the school physician will conduct examinations and notify parents of conditions requiring further attention.

The New York State Department of Health now requires that students entering sixth grade must receive a tetanus, diphtheria, and pertussis booster (Tdap)

  1. Proof of Residency (One of the following):
  • Mortgage/Paid Tax Bill document for residence

  • Notarized Rent/Lease document for residence

•  If the mortgage statement or lease is not in your name, a landlord affidavit form will have to be filled out and notarized

IMPORTANT: All renters and lessees must attach a copy of the deed or mortgage statement or paid tax bill.

Lastly, any two of the following documents must be submitted:

  • Driver's license of parent/guardian

  • Telephone bill of residence (no cell phone bill)

  • Utility bill of residence, i.e., electric, oil, cable, etc.

  • Most recent income tax document

  • Updated voter's registration card
